Vasectomy is for men who consciously and thoughtfully choose permanent protection against conceiving a child - usually those who already have the number of children they planned, or who have deliberately decided not to have children.
Vasectomy is treated as a permanent method, which is why we discuss the decision calmly during a qualification consultation. It is not a decision made on the spot.
What problem it solves
Vasectomy is one of the most effective methods of contraception. It involves interrupting the vas deferens so that sperm no longer enter the semen.
The procedure does not affect testosterone levels, libido, erection or the sensation of orgasm.
The contraceptive effectiveness of vasectomy is around 99.95% - the risk of pregnancy is roughly 1 in 2,000. The failure rate of the technique itself is below 0.5%. Spontaneous recanalisation of the vas deferens is possible, including years after the procedure.
How qualification works
During the qualification consultation we discuss your motivation and expectations, the permanent nature of the procedure, how it is performed and the possible risks, as well as how effectiveness is verified afterwards.
This is the time for all your questions - including the ones that feel awkward. Qualification takes place in person, not online.
How the procedure works
The procedure is performed on an outpatient basis. I gain access through a few millimetre-wide openings in the scrotal skin, made without a scalpel (no-scalpel vasectomy, NSV). Everything is done under local anaesthesia and usually takes about 20–30 minutes.
The vas deferens are then interrupted and secured: I seal the prostatic end with coagulation, the testicular end is either left open or sealed, and both ends are always placed in separate tissue planes (fascial interposition). This is known as the Canadian technique.
You go home the same day.
Recovery
You can usually return to office work after about 2 days. Physical exertion and sexual activity are possible after 7–14 days, depending on how healing progresses.

Possible side effects and limitations
Like any procedure, vasectomy can involve complications:
- haematoma
- wound infection
- epididymitis
- painful granuloma
- chronic scrotal or testicular pain - significantly affecting daily life in 1–2% of patients
- very rarely, testicular atrophy
- a reaction to the anaesthetic
Complications are rare and usually mild; I discuss the details during qualification.
The contraceptive effect is not immediate. The follow-up semen analysis is done after about 12 weeks and at least 20 ejaculations (the AUA guideline gives a range of 8–16 weeks). Additional contraception is required until effectiveness is confirmed.

Alternatives
The alternatives are other methods of contraception, including reversible ones. If plans change, microsurgical vasectomy reversal (vasovasostomy) is an option - I perform this procedure as well - but the success of a reversal is not guaranteed. That is why vasectomy should be treated as a permanent decision.

How should I prepare for the procedure?
Detailed instructions are given individually during the qualification consultation. It is worth planning a few quieter days afterwards and preparing comfortable, well-fitting underwear.

Can vasectomy cause permanent pain?
Chronic pain that significantly affects daily life occurs in 1–2% of patients and usually responds to treatment. The technique I use - leaving the testicular end open and using fascial interposition - is intended to minimise that risk.
Does it hurt?
The procedure is performed under local anaesthesia; discomfort is usually minor and short-lived.
